- Both new and returning clubs MUST complete this form to be an active UNICEF Club.
- We require the contact info of 4 club leaders and 1 advisor to form a club. Collect this info before you begin! There is no save and return option.

- Once your registration is approved by our team, you will receive an approval email (within 7-10 business days), including links to helpful resources. You'll even have the chance to start winning UNICEF USA merch!
